What Is Rhinophyma?

Rhinophyma is the rarest, most severe type of rosacea. It causes oil glands on the nose to become overactive, with a combination of inflammatory cysts, whiteheads, and blackheads. 

The tissue can become thickened and bumpy, with prominent blue or red vessels visible on the skin’s surface. Sebaceous glands expand, causing tissue to overgrow, resulting in an enlarged, often disfiguring nose that may even affect the ability to breathe properly.

What Is Rosacea?

Rosacea is a common inflammatory disorder that can cause redness, flushing, dry skin, and prominent vessels on the face. 

It can also be associated with small bumps or pimples that can develop on the nose and cheeks, along with symptoms of burning and stinging. These issues are classified as erythrotelangiectatic or papulopustular rosacea. 

Symptoms of rosacea can be treated with a combination of topical medications, oral medications, and vascular laser treatment. 

What Causes Rosacea?

The cause of rosacea is unknown. There may be a genetic componentand, and it usually only occurs after age 30

Symptoms can fluctuate, with flare-ups relating to specific triggers such as heat, sun, spicy foods, alcohol, stress, and exercise. 

There is no cure for rosacea, so our goal is to manage the condition by identifying and avoiding triggers. We recommend a combination of prescription medications, medical-grade skincare, daily SPF, and intermittent in-clinic procedures. 

Treating Rhinophyma

In the past, plastic surgery procedures with a scalpel or electrocautery were the main methods used to treat rhinophyma. Unfortunately, these methods frequently resulted in scaring or limited improvement.  

As the primary underlying condition is excessive and enlarged oil glands, the UltraPulse Co2 Laser has revolutionized the treatment of this condition. The laser vaporizes the unwanted skin, shrinking excess oil glands while allowing the physician to reshape the nose.

In a single session, the excess tissue can be removed, and we can restore the appearance and function of the nose.

What Happens During Laser Treatment?

Before the procedure, oral sedation is administered, and then the area is frozen with a local anesthetic. We use the laser to vaporize the excess tissue one layer at a time until the desired endpoint. Immediately after the procedure, your nose may appear dark, but typically, there is minimal to no bleeding. 

You’ll notice some crusting, and postcare involves keeping the area moist and gently cleaning away the damaged tissue a few times a day—no need for any dressings. Most of the crusting and peeling should clear up in about 2–3 weeks.

Laser Treatment: Enhanced Improvement & Long-Term Maintenance

As rosacea is a chronic condition that can get worse over time, long-term maintenance with a combination of topical and occasionally oral medications can help control excess oil production. 

We suggest adopting a medical-grade skincare routine. Vascular lasers, which can help with noticeable veins and blemishes, are recommended every 6–12 months. By following this recommended maintenance protocol, we rarely need to redo major Co2 laser procedures.
